Yes! Please choose a research site which is most convenient for you.
Yes! You will need to log in and change your selected date. Please ensure this is still within the right time frame (T0 = within 6 months from diagnosis, T1 = 6 months from T0, T2 = 12 months from T2; these dates have +/- 2 weeks leeway either side.
Your GP should appear on the drop down list. If it does not, the study has not reached your GP practice yet but please come back and check again soon!
If you live in Hampshire or Dorset please select SDSC Path, as your individual GP practice will not be present.
